Outstanding contributions to family medicine by the staff of the West Carleton Family Health Team made possible by the early adoption of an EMR system

Family Practice of the Year Award
The Family Practice of the Year award was introduced in 2008 by the Ontario College of Family Physicians (OCFP) to provide an opportunity to celebrate the accomplishments of the members of a family practice. Family doctors, other members of the practice team, and health care professionals from other organizations may nominate any family practice for the award, including their own. The OCFP chooses a family practice based on criteria such as the skill of the clinicians, the resources for the patients, the presence of the practice in the community, and the relationships of the physicians to their patients.
