Simply click on the custom form you want, CIS fills in the information it can, and you check off the information specific to that encounter. Not only do you have a quick form that is ready for the patient or the outside party, but you have automatically created a record of the form in the patients chart.

Custom Forms is fully integrated into P & P’s electronic medical record. Instead of using our generic diagnostic referral sheet, attach your local x-ray clinics form and fill it out online. Create tracking forms or download one off the internet, and when it comes time to refer the patient, you can seamlessly attach the forms you have used to the letter.
